THE SHORT VERSION
A nationally recognized facial plastic surgery practice with three service lines and an accredited surgery center is hiring its first true operations leader. The seat reports directly to the owner, carries every non-clinical function in the practice, and is being created — not backfilled — with a defined structure already built for whoever takes it. If you have been running a practice without the title or the authority, this is the step up.
The opportunity
Our practice is a physician-owned, facial plastic surgery practice, led by an internationally published facial plastic surgeon and a recognized authority in hair restoration and facial aesthetics. The practice runs three distinct businesses under one roof — surgery, hair restoration, and aesthetics/injectables — supported by an accredited ambulatory surgery center and a team of roughly 15–19.
The practice is profitable, established, and growing. What it does not yet have is a single operations leader. Ownership has deliberately created this seat rather than distributing the work and has invested in defining it properly: the organizational design, reporting structure, scorecard, and first-90-days plan are already built. You are inheriting a structure, not improvising one.
What makes this different from most practice manager jobs
Almost entirely cash-pay
Elective aesthetic and surgical services. No payer contracts to manage, no denials queue, no credentialing treadmill. Your energy goes into the business, not into insurance.
Three businesses, not one
Surgery, hair restoration, and aesthetics each have their own economics, funnel, and rhythm. You will manage a portfolio, not a department.
Real authority
Front office, patient experience, operations and facilities, finance operations, HR, vendors, and compliance all roll up to this seat. You will have direct reports and budget visibility.
An owner who wants to step back
Ownership is actively working to remove itself from daily operations. Your success is measured partly by how little you need them.
Structure already in place
A documented org design, a defined scorecard, an SOP program, and a role-tiered performance dashboard are being stood up now. You inherit the instrumentation.
What you will own
- Daily operations across all three service lines — schedule integrity, room and OR utilization, patient flow, and the coordination rhythm between clinical, front office, and operations.
- The team — hiring, onboarding, scheduling, performance management, and development for the non-clinical staff, in partnership with the clinical leadership.
- Financial operations — oversight of daily reconciliation, accounts payable and receivable, payroll processing, and vendor contracts, working alongside a retained fractional CFO who provides independent review.
- Business performance — consultation-to-treatment conversion, lead follow-up discipline, case volume, and the metrics that drive each service line, reviewed weekly against a scorecard.
- The patient experience — the standard from first inquiry through post-treatment follow-up, including reputation and review management.
- Facilities, compliance, and risk — surgery center accreditation support, OSHA and HIPAA program administration, equipment and vendor management, and the operating policies that keep all of it current.
Who we are looking for
We are far more interested in how you operate than in where you have operated. If you have run the business side of a high-volume, consultation-driven, cash-pay practice, we want to talk to you — regardless of specialty.
Backgrounds that translate well
- Facial plastics, plastic surgery, or cosmetic dermatology practice operations
- Medical spa or aesthetics group management, including multi-location
- Refractive surgery / LASIK, oculoplastics, or cosmetic ophthalmology
- Orthodontic, cosmetic dental, or oral surgery practice management
- Fertility, concierge medicine, or another elective, deposit-driven service line
- Specialty veterinary or another high-touch, private-pay clinical business
- Luxury retail, hospitality, or service general management — paired with clinical-environment exposure
What we expect you to bring
- Five or more years managing the operations of a clinical or high-touch service business, including direct supervision of a team
- Real ownership of a P&L or an operating budget — you can read a financial statement and explain a variance
- Command of a practice management or EHR platform and comfort building reporting out of it (we use DrChrono; platform-agnostic fluency matters more than that specific system)
- A track record of turning informal habits into documented process, and getting a team to actually follow it
- The judgment to make decisions in the owner’s absence, and the discipline to escalate the ones that warrant it
- Bachelor’s degree preferred; CMPE, FACMPE, or CMOM credentials welcome but not required
BE HONEST WITH YOURSELF ABOUT FIT
This is a hands-on seat in a small organization. There is no layer beneath you to absorb the work, and on a hard day you will be solving problems yourself rather than assigning them. It suits an operator who wants scope and autonomy — not one who wants a large department and a defined lane.
Compensation and structure
- Base salary: [$XXX,XXX – $XXX,XXX], commensurate with experience.
- Performance incentive tied to a defined operational and financial scorecard, reviewed [quarterly].

- Reports to: Owner / CEO. Works closely with the Co-Owner on strategy and governance, and with the retained fractional CFO on finance.
